    I doubt you will find one.

    Most companies use a form that is like any application you complete.

    ie;
    name, address, your car, other car, driver names, passengers, injuries, police called, police #, date of accident, time, location etc.

    then they usually a space that states tell me what happened. it will usually have about 4 or 5 lines that you would write, going EB on main, other driver SB, came to a stop sign and other driver pulled out in front of me causing accident.

    they may have another diagram on it, where you can show your car and the other.

    then the bottom asks that you sign and date.

    that is just the 1st step. the adjuster will then take recorded statements of each party. will get estimates of the damages, get a copy of the police report, and copies of medical bills/records if anyone is injured.

    this process can take a few days or months to finalized depending on the type of claim.

    you might want to try the library to see if there is a book with sample claim forms, like legal forms. otherwise, even with a google search most likely will get all kinds of advertisement for lawyers, auto insurance etc.

    good luck

    http://www.AutoInsuranceEstimate.info Has a form you just fill in and it shows you different rates from different auto insurance companies. Go with which one is the least expensive. That’s what I did.

    autoinsurance.bebto.com – try this one. I have their car insurance and, as I know, they can provide such a service.
